Screening Tools

The EIR program conveniently offers 3 tools capable of identifying and predicting emotional/behavioral health symptoms of children ages 3 months- 21 years of age.

1) The Ages & Stages Questionnaire: Social-Emotional (ASQ:SE) for children ages 3 months- 5 years 5 months. http://agesandstages.com/asq-products/asqse/

2) Strengths & Difficulties Questionnaire (SDQ) Parent or Teacher version: for children ages 6 years-17 years http://www.sdqinfo.com/a0.html

The Ages & Stages Questionnaire can be completed by a parent, guardian, care taker, or day care provider. The Strengths and Difficulties Questionnaire, depending on the child's age, is completed by just the parent, by both the parent and the child. Parents/Guardians can provide written permission for teachers to complete screenings as well.
